Reports we offer
Our speech and language therapists offer a range of reports for individuals with speech, language and communication difficulties. Our speech and language therapists also offer specialised reports for individuals with more complex needs.
Our speech and language therapists offer reports for the following reasons after an initial assessment to provide evidence of assessment, this can provide input for diagnosis of conditions or can provide justification and help therapists choose appropriate treatment programmes.
We also provide review reports, these are for patients that are having ongoing therapy to review their progress or for patients who have finished a treatment programme who have returned for a review of their speech, language and communication difficulties. Discharge reports are also offered to document each patient's therapy progress and to justify discharge.
Specialised reports can also be provided by our speech and language therapists upon request, these include; school transition reports, assessment of capacity, Education, Health and Care plan reports and medico-legal reports. These reports will be tailored to your requirements after discussion with one of our speech and language therapists.
Reports can also be written by our speech and language therapists if our patients require specialist services off other health professionals or specialist equipment including, Alternative and Augmentative Communication (AAC).
What will reports involve at SLT UK?
The content of a speech and language therapy report will vary depending on the type of report and also the individual needs of the patient. A speech and language therapy report will provide a detailed overview of the patient’s needs and abilities. A report will also include assessment results and recommendations made by the speech and language therapist.
A speech and language therapy report will provide recommendations for parents, carers, teachers and other professionals involved in the patient’s care, in ensuring the patient receives the best possible outcome regarding their speech, language and communication needs.
